Inspection of the Hongfeng Coal Mine in Fugu County revealed violations.
1. The wind measurement station in the northern return airway of the first level second panel did not have a wind speed sensor installed;
2. The belt conveyor tail at the excavation face of the second level auxiliary transport roadway lacked lighting; the head of the belt conveyor in the 12111 fully mechanized mining face did not have protective barriers; there were two observation stations in the 12211 fully mechanized mining face transport channel where pedestrians crossed the belt conveyor without a bridge;
3. The support on the non-pedestrian side of the second level auxiliary transport roadway excavation face was lagging 80m from the heading, which does not comply with the operational regulations that state it should not exceed 50m; there were two anchor rod trays at the end of the 12211 fully mechanized mining face transport channel that were not flush with the rock surface; multiple areas in the 12207 intake airway showed delamination or spalling, and support was not reinforced;
4. Two low-voltage cables suspended from the roof near the head of the belt conveyor in the second level auxiliary transport roadway were laid in a crossed manner, with no spacing;
5. The power supply lines for the butterfly valves of the main fans 1# and 2# were drawn from the main fan I circuit, lacking a backup power supply that is equally reliable as the main equipment;
6. The local grounding electrode length set at the distribution point of the belt conveyor head in the 12211 fully mechanized mining face was 1.5m, with 0.1m exposed above ground, and was not fully vertically buried in the base;
7. On September 3, 2025, on-site testing of the gas electric lock in the second level auxiliary transport roadway excavation face showed that after the gas exceeded the limit alarm, the personnel location monitoring system did not respond; on-site testing of the wind duct sensor gas electric lock in the second level auxiliary transport roadway excavation face showed that the personnel location monitoring system and voice broadcast system did not trigger an alarm;
8. The mine did not inspect the external fixed cables in July and August 2025;
9. The fire door wall in the transport channel of the 12211 fully mechanized mining face had cables passing through, and there were no signs indicating the number, purpose, voltage, and cross-section on both sides of the wall-penetrating cables;
10. Review of the mine safety monitoring system showed that on August 28, 2025, the wind speed sensor at the second level return dark inclined shaft displayed values less than 2m/s throughout the day, while the wind speed board showed a measured wind speed of 2.42m/s, exceeding the allowable error (0.3m/s) stated in the sensor manual, and was not addressed in a timely manner;
11. Review of industrial video footage showed that during the support operation in the second level auxiliary transport roadway on August 30, 2025, from 13:40 to 14:00, the temporary support of the fully mechanized mining machine was not raised to the roof;
12. The 16# trackless rubber-wheeled vehicle parked at the second level return dark inclined shaft had no water in the explosion-proof water tank;
13. The three 3kg carbon dioxide fire extinguishers used in the surface main fan room distribution room were not checked by the weighing method as required by the manual, making it impossible to determine their condition;
14. During the operation of the cutting part of the excavation machine in the second level auxiliary transport roadway excavation face from 13:30 to 13:40 on August 30, 2025, personnel were standing between the machine body and the coal wall;
15. The wind duct joint at the first branch of the second level return roadway was leaking air; the pressure air pipeline at 620m in the second level auxiliary roadway was leaking air;
16. The pressure sensor of the left column of the 111# hydraulic support in the 12211 fully mechanized mining face was faulty, showing an uploaded pressure of 0, while the actual pressure on-site was normal;
17. The mine did not equip emergency supplies according to the emergency plan requirements; the plan required three submersible pumps in the surface fire material warehouse, but only two were actually equipped; the plan required one magnetic starter in the underground fire material warehouse, but none were equipped;
18. Used cotton and cloth scraps were temporarily stored at the return air inlet of the central substation, not stored in a tightly sealed iron bucket.

The Hongfeng Coal Mine in Fugu County (general partnership) was established on October 10, 2008, located in Guopeilin Gully, Laogaochuan Township, Fugu County, Yulin City, Shaanxi Province, with the legal representative being Shi*bing. The mining area is 5.8km long from north to south and 2.5km wide from east to west, covering an area of 11.0855km2, with a designed annual output of 900,000 tons.
The mine adopts a gentle slope inclined shaft development method, a long-arm fully mechanized coal mining method, and manages the roof using the caving method. The mine ventilation uses a central parallel ventilation system with an exhaust ventilation method.
