8 Clinical Significances of Elevated C-Reactive Protein Beyond Bacterial InfectionC-reactive protein (CRP) is an acute-phase response protein induced by interleukin 6 (IL-6) produced in liver cells, with normal reference value < 5 mg/L. The advancement of testing technology has improved the sensitivity and accuracy of CRP detection. Existing testing instruments can detect extremely low concentrations of CRP in serum samples, known as high-sensitivity C-reactive protein (HS-CRP).When the body is infected or tissue is damaged, CRP rises sharply in plasma, functioning by activating complement and enhancing phagocytosis to eliminate invading pathogens and damaged, necrotic, or apoptotic tissue cells. Therefore, it plays an important protective role in the body’s innate immune process. CRP is a nonspecific inflammatory marker, and its elevation cannot simply indicate bacterial infection. The trial showed that preoperative CRP ≥ 3.1 mg/L and postoperative peak CRP (CRPmax) ≥ 77.1 mg/L are both risk factors for RFS [8].It is important to note that CRP can elevate during the active phase of malignant tumors. Therefore, routine follow-up of CRP in patients with malignant tumors can assist in assessing the progression of their condition.Acute PancreatitisIn severe acute pancreatitis patients, serum CRP concentrations can reach ≥ 200 mg/L, and the severity of the condition is positively correlated with CRP levels. Using a CRP level of 210 mg/L as a reference for assessing the severity of acute pancreatitis, its sensitivity is 83% and specificity is 85% [9].ConclusionThis article summarizes the pathological mechanisms of CRP as a biomarker in infection or inflammatory responses and discusses its clinical value in inferring infectious pathogens and assessing infection severity. CRP’s specificity is not high; both infectious and non-infectious diseases can cause its levels to rise, but the rise and fall of CRP levels have guiding significance for clinical practice.
