02Harmonious Co-construction and Agricultural Development
To effectively meet the needs of individuals undergoing home rehabilitation treatment, Zhengzhou Beixiajie Community Health Service Center fully utilizes the role of family doctor contract services, deploying family doctor team members into community grids to carry out various forms of “embedded” services, enhancing interaction with community residents and safeguarding their health.
1. Family Doctor Team Joins Community WeChat Groups
Community WeChat groups are generally established based on residential complexes or buildings. Each community’s family doctor team needs to join more than ten WeChat groups, actively identifying themselves as healthcare professionals, sharing contact information, and establishing direct communication with residents. They provide information on COVID-19 treatment methods, rehabilitation plans, expert interpretations, medication guidance, psychological counseling, and advocate for healthy lifestyles such as balanced diets and moderate exercise, encouraging residents to adopt a positive mindset in response to the COVID-19 pandemic.
2. Family Doctor Team Integrated into Community Grid Services
Family doctors actively engage in secondary grids, posting healthcare information boards in each community and building, displaying the names, contact numbers, specialties, and responsibilities of healthcare personnel, providing timely medical assistance and guidance to residents, especially those undergoing home rehabilitation treatment.
3. Printing Family Doctor Service Contact Cards and COVID-19 Rehabilitation Treatment Pamphlets
Increasing publicity efforts, aiming for one card and one booklet per household. Together with community volunteers, home service is provided to those undergoing rehabilitation treatment at home who meet the conditions and have genuine needs, maximizing convenience for residents.
4. Strengthening Fever Clinic Staffing
First, increase the number of clinics; second, enhance the staffing of ward physicians to the outpatient front line, fully receiving fever patients, prescribing symptomatic medications for mild cases, alleviating discomfort, providing reassurance, and implementing a first-visit responsibility system.
5. Establishing a Green Channel for Referral of Severe and Emergency Patients
Strengthening preventive guidance and health monitoring for vulnerable groups such as pregnant women, children, and the elderly. In case of emergencies, promptly assist in contacting and referring them to higher-level hospitals for treatment.
6. Establishing an Emergency Duty System
In addition to normal duties in various departments, an emergency duty team for home rehabilitation treatment is formed, consisting of ward and outpatient healthcare personnel, with two doctors and two nurses on emergency duty daily, ensuring continuous service at night and guaranteeing that home rehabilitation patients have guidance and support at all times.
7. Enhancing Health Education
All promotional materials and electronic screens within the institution are adjusted to focus on COVID-19 treatment and rehabilitation guidance. Clinical department doctors provide individualized health guidance and psychological counseling to patients through information push and phone follow-ups. Strengthening community publicity to guide mild and asymptomatic infected individuals to undergo home rehabilitation treatment, showcasing the capacity of grassroots medical services, informing about the risks of medical resource strain, and ensuring that medical resources are reserved for those truly in need, such as the elderly, sick, and severely ill patients.
The Beixiajie Community Health Service Center consistently adheres to its function of providing basic medical and public health management services to community residents, utilizing the “embedded” service approach to quickly integrate into grid management, providing guidance on disease treatment and prevention, effectively achieving the integration of medical and preventive care, ensuring the safety and health of the community residents, and serving as guardians of community health!
