ESC 2024: PROTEUS Study on AI in Stress Echocardiography
From August 30 to September 2, 2024, the European Society of Cardiology Annual Congress (ESC 2024) will be held in London, UK, and online globally. During the HOT LINE 12 session, Professor Ross Upton from the University of Oxford will announce the results of the PROTEUS study. Research Background

Stress echocardiography (SE) is one of the most commonly used diagnostic imaging tests for coronary artery disease (CAD), requiring clinicians to visually assess the scanned images to determine which patients may benefit from invasive testing and treatment. EchoGo Pro can automatically interpret SE based on artificial intelligence (AI) image analysis. In reader studies, using EchoGo Pro for clinical decision-making improves diagnostic accuracy and reliability. To understand the impact of EchoGo Pro on patient treatment pathways and outcomes, a prospective evaluation in real practice is necessary. The PROTEUS study (NCT05028179) analyzes whether the intervention (EchoGo Pro) combined with standard care is not inferior to standard care alone in referring patients for coronary angiography after SE examination..

Research Methodology

PROTEUS is a randomized, multicenter, two-arm, non-inferiority study aimed at recruiting 2,500 participants from National Health Service (NHS) hospitals in the UK who will be referred for SE to evaluate suspected CAD. All participants are randomly assigned in a 1:1 ratio to either the control group representing current practice or the intervention group, where clinicians will receive an AI image analysis report (EchoGo Pro, Ultromics Ltd, Oxford, UK) for image interpretation, indicating the likelihood of severe CAD. Two remote follow-ups will be conducted at 3 months and 6 months post-randomization, including medical record reviews (conducted by the research team) and two patient-reported questionnaires.

The primary endpoint is the appropriateness of the clinician’s decision to refer for coronary angiography, which will be assessed 6 months later based on a review of participants’ records. Secondary endpoints will evaluate other health impacts, including appropriate use of other clinical management approaches, the impact on decision variability, qualitative experiences of patients and clinicians, and health economic analysis.

Research Progress

The study began recruitment in September 2021 and is expected to continue until December 2023. This will be the first study to assess the impact of introducing AI diagnostic decision support tools into the standard care pathway for suspected CAD patients undergoing SE. The results of the PROTEUS study will provide an important opportunity for economically effective reforms to standard treatment pathways, leading to positive treatment outcomes for patients..
